​Changing the specialist mental health care:

The concentrated treatment format

Patients with anxiety disorders and with depression are by far the most common, the most frequently seen and the ones who receive the longest treatment courses in the psychiatric outpatient specialist health care.

More than 3/4 develop their disorder before their midtwenties, and without proper treatment, the conditions tend to become chronic, as reflected in the overrepresentation of these patients among in the number of young disability cases. Obviously the socio-economic gains are immense for each patient who can be helped. The group behind this application has written a story of success in mental health care in Norway: We have been able to initiate and sustain change simultaneously on three levels:


Treatment content, treatment format and dissemination

This has basically been accomplished by reallocating existing resources effectively and simultaneously enhancing competence in the health system. Starting with Obsessive Compulsive Disorder, we have developed and tested an awarded concentrated treatment format (CTF), delivered during 4 consecutive days. The approach is highly accepted by the patients, and there is basically no drop-out. Response rate is 86% posttreatment and 83% at 6 month follow-up. The corresponding remission rate is 75% and 68%, respectively The direct treatment cost of this approach is reduced to approximately 50% as compared to standard evidence-based OCD-treatment. So far more than 400 patients have received the CTF.

Primary and secondary objectives

Change the Norwegian Outpatient Specialist Mental Health Care by the 4-day format
A. IMPROVE TREATMENT GAINS BY CHANGING CONTENT AND FORMAT OF THE HELP
OFFERED: Test the feasibility and effectiveness of the CTF for patients with anxiety disorders
and depression by comparing treatment satisfaction and clinical gains for patients
randomized to the CTF or to treatment as usual (TAU)
B. REDUCE TREATMENT COSTS AND INCREASE SOCIO-ECONOMICAL TREATMENT
GAINS: Compare direct and indirect treatment costs and gains for patients randomized to
CTF or to TAU .
C. MAKE THE HELP AVAILABLE AND SUSTAINABLE: Train and certify approximately 30 CTFteams
D. GAIN BASIC KNOWLEDGE OF PLASTICITY: Explore epigenetic and psychobiological
predictors and changes
